My child has permission to participate in More Mirrors LLC programs. I waive any right to claim against More Mirrors LLC owners, staff, and teachers in the event of an accident, injury, or loss of personal items. I am aware that the dance teacher is not an official employee of the preschool center but will provide instruction and services to my child and will not be under the direct supervision of preschool staff. In the event photographs are taken which include my child, I grant More Mirrors LLC the right to use photographs and/or likeness of my child in the promotional materials including, videos, photos, social media and display boards. I understand there will be no compensation to myself or my child. I agree to pay tuition in advance for my child's enrollment.
